Many hospitals and other facilities require a written release form
before we can remove your loved one from the place of death. This is
signed by the legal next of kin, Power of Attorney for Health Care or
the one designated in the Advance Health Care Directive. This form can also be printed prior to the death to keep in the medical records folder for when it is needed.
